View and download a formulary.

A formulary, also known as a drug list, shows plan members the prescription medications which are covered under the health care plan's prescription drug coverage.

You can look up your drugs in the formularies based on your plan's coverage region listed below. The formulary may change during the plan year. If you view the formulary, also review formulary additions, formulary deletions, step therapy, and prior authorization criteria.

If you would like a hard copy of the formulary mailed to you, please contact Customer Service here or call toll-free 1-800-650-6232, TTY 711. Customer Service hours: October 1 - February 14: 8 a.m. - 8 p.m. local time, 7 days a week. February 15 - September 30: 8 a.m. - 8 p.m. local time. On Saturday, Sunday and holidays, please leave a detailed message and a representative will return your call within one business day.

Coverage limitations: To be covered, drugs must be prescribed for a use that is approved by the FDA or documented in at least one of the specific peer-review materials identified by the Centers for Medicare and Medicaid Services (CMS). You can find out if any additional prescription drug coverage limitations apply to your drugs by looking at the prior authorization documents. Prior authorization requires you or your doctor to get approval from the plan before your drug is covered.

The Formulary, pharmacy network and/or provider network may change at any time. You will receive notice when necessary.
